Requested-command output is rendered by the inline pane whenever it is
expanded. Keep the backing terminal block hidden in both states so
collapsing the pane does not move the output to the bottom of the
conversation.
- Track user-initiated expansion state to avoid auto-collapse conflicts
- Invert inline action visibility so terminal block hides while inline pane owns the expanded body

Move token/cache/cost metrics from the expandable per-block usage button
into a persistent status bar rendered in the agent input area. This gives
always-visible feedback without requiring user interaction.

- Persist cache/cost totals in ConversationUsageMetadata for session restore
- Remove render_usage_button, ToggleIsUsageFooterExpanded action, and
UsageFooterToggled event

- Fix has_footer() to return false so blocks no longer reserve phantom
footer space (root cause of padding/margin issue)
